STUDENT IDENTITY VERIFICATION
(SIV)
While taking your course online, and during the online final examination, you will be asked Student
Identity Verification (SIV) questions randomly. These questions are taken from the information
that you provided during your enrollment. You will have 45 seconds to respond
correctly to these questions.
During Your Course:
you will be asked random SIV questions. If you miss two
consecutive SIV questions you will be denied access to your
course and you will need to
contact your school.
IMPORTANT:
If you are denied access more than once (due to missing the required number of SIV questions), your
course will be permanently disabled; therefore, you will need to
contact your school. The School's Administrator will have to RESET your
course back to Lesson 1 (once you have successfully proven your identity by answering questions by the School's Administrator).
During Your Online Examination:
you will be asked random SIV questions. If you miss ANY SIV question
during the online final examination, you will be denied access to the exam and must
contact your school to schedule a paper and pencil examination. Once you start the online final examination
you must
complete the exam in one sitting.
STUDENT AFFIDAVIT (Alternative Instructional
Methods Reporting Form)
The Student Affidavit is a signed document
certifying that you have personally
completed each assigned lesson of instruction. It is a REQUIREMENT for
course completion, and must be returned to United Homeland or you will NOT be eligible
for
course
completion (which means that you will NOT receive a Certificate of Completion and/or have your
course
completion reported to your state). In addition, you will also be required to complete a
Product and Customer Service Evaluation questionnaire before
completing your Student Affidavit. This questionnaire is
completed online and will automatically appear once you
click on the Student Affidavit/MCE Reporting Form link that becomes available on your "Course Home Page" after you have
completed the last lesson of your
course.
